Thank you for trusting CarePlus with your health care for 2026
We have news about your CarePlus Health Plans® coverage. As you may know, CarePlus is part of the Humana family. As of Jan. 1, 2027, at the direction of the Centers for Medicare & Medicaid Services (CMS), CarePlus and Humana will no longer be able to both offer a D-SNP product for full dual-eligible enrollments in the same county.
Full dual-eligible members like you affected by this change who do not elect a different plan during the Annual Enrollment Period (AEP) will be transitioned to a Humana D-SNP for full dual eligible members as of Jan. 1, 2027.
Your Annual Notice of Change (ANOC)
Your ANOC includes information about the Humana D-SNP in which you’ll be enrolled as of Jan. 1, 2027. Please review the information in the ANOC carefully, so you can compare your plan benefits for 2026 (your current CarePlus benefits) and for 2027 (the Humana benefits you will receive) side by side. The ANOC includes important changes but does not include a full list of all plan benefits.
To access your 2027 ANOC:
- Log in to your secure MyCarePlus account, or if you do not have a secure MyCarePlus account, create one in minutes.
- You will be able to continue accessing your secure MyCarePlus account through December 31, 2027.
- You can use your same MyCarePlus logon credentials to access your Humana account at
MyHumana.com . - You can also access your 2027 ANOC in the plan benefit document listing below, which is available in English, Spanish, and Creole.
Other important information
Your member ID number, which begins with a letter H, will remain your member ID number, even after Jan. 1, 2027. You will get a new Humana member ID card with your member ID number on it. Please use this new member ID card on and after Jan. 1, 2027.
Many of our in-network doctors, like primary care physicians (PCPs), accept CarePlus and Humana coverage. We are already working to contract with PCPs who may accept CarePlus but not Humana, to make sure you can continue seeing your PCP after Jan. 1, 2027. You always can ask your PCP if they accept Humana, as well. If your PCP does not contract with Humana, you can pick a new PCP, or we will assign you one. The PCP’s information will be on the new Humana member ID card that you get.
Also as of Jan. 1, 2027, your care manager, if you currently work with one, will change. Your new care manager will contact you before Mar. 31, 2027, to complete your health risk assessment.
Before you transition from CarePlus to Humana on Jan. 1, 2027, we strongly encourage you to:
- Review the Humana provider directory (or ask your doctor) to make sure the doctors you see now are in the network. If they are not listed, it means you will likely have to select a new doctor.
- Review the Humana pharmacy directory to make sure the pharmacy you use for any prescription medicines is in the network. If the pharmacy is not listed, you will likely have to select a new pharmacy for your prescriptions.
- Review the formulary (Drug List) to make sure your drugs are covered.
