Dental coverage is included on every CarePlus plan. If you are a CarePlus member, read this page for important dental benefit information and updates.

Important Dental Provider Network Update: Effective Sept. 1, 2023, our new dental provider network is now Florida GoldPlus Dental, administered by HumanaDental Insurance Company. Please read below for additional information and next steps for members.

Dental Network Update Details

We changed our dental network to help make sure you receive the best possible service. However, we understand you may have questions. Below, you can find important information and steps to help with the transition. 

What does this change mean?

  • This dental network change does not affect your covered plan benefits.
  • More dental providers are in our new network, offering you even more options when seeking care.
  • You will have a new dedicated dental member ID number.
  • Prior authorization and referrals are no longer required.

What actions do I need to take?

  • Save the dental member ID information that was mailed to you to use at all dental visits.
  • Use the Dental Finder tool to check if your dentist is participating with Florida GoldPlus Dental.
  • When scheduling a dental appointment, we encourage you to state the following: “I have dental benefits with my CarePlus Medicare Advantage plan. The dental benefits are provided by dentists in the Florida GoldPlus Dental Network.”
  •  Register at MyHumana.com using your new dental member ID to view dental benefits and dental claims and to access the dental provider search. You will still use MyCarePlus, your secure member portal, to access all other health plan information, including benefits, medical claims, and more.

How to File a Dental Grievance or Appeal

You can submit your grievance or appeal within 60 calendar days of the date of the event or denial notice. Your grievance or appeal should be sent in writing; however, we’ll accept phone submissions. Please call Member Services at 1-800-794-5907 (TTY: 711) or mail/fax your request to:

How to ask us to pay you back or to pay a bill you have received:

You may request us to pay you back by sending us a request in writing. If you send a request in writing, send your bill and documentation of any payment you have made. It’s a good idea to make a copy of your bill and receipts for your records. You must submit your dental claim to us within 12 months of the date you received the service or item.

To make sure you are giving us all the information we need to make a decision, you can fill out our claim form to make your request for payment.

You don’t have to use the form, but it will help us process the information faster. Send us your request for payment along with the following information:

  • Proof of payment– Itemized bill listing the item or service received
    • Dentist order (if applicable)
    • Dental records (if applicable)
    • Any other supporting documentation
