How to Enroll in a CarePlus Medicare Advantage Plan

CarePlus Online Enrollment is Secure, Quick, and Convenient

Completing the enrollment process into a CarePlus Medicare Advantage Plan is easy and convenient. We keep all information you share with us strictly confidential and secure. You can enroll online 24 hours a day, 7 days a week during Medicare’s Annual Enrollment Period (AEP). Enrollment outside AEP is limited to those who meet certain conditions or qualify for a Special Enrollment Period (SEP). Contact CarePlus at the phone number below for more information.

Enroll Online

Our CarePlus Medicare Advantage Plan online enrollment tool is fast, secure, and easy. You can access our online enrollment tool on our homepage.

  1. Enter your ZIP code to compare CarePlus Medicare Advantage plans, except for Special Needs Plans (SNPs), available in your area. If you are interested in a SNP, you can call the number below to see if you’re eligible and to enroll.
  2. Use our online plan comparison tools to compare drug costs, plan benefits, and more.
  3. Begin the quick and simple enrollment process, once you have found a plan that works for you.

Medicare beneficiaries also may enroll in CarePlus through the Centers for Medicare & Medicaid Services (CMS) Medicare Online Enrollment Center located at http://www.medicare.gov (link opens in new window) .

Enroll by Phone

To enroll by phone, call a licensed CarePlus sales agent at 1-855-605-6171, (TTY: 711). From October 1 - March 31, we are open 7 days a week, 8 a.m. to 8 p.m. From April 1 - September 30, we are open Monday - Friday, 8 a.m. to 8 p.m. You may always leave a voicemail after hours, Saturdays, Sundays, and holidays and we will return your call within one business day.

On each form below, we include instructions for completing and mailing the Enrollment Form. The enrollment application for the 2023 plan year is available beginning October 15, 2022.

Please note: In general, your coverage begins on the first day of the month after the month we receive your completed enrollment form. However, if you change plans during the Annual Enrollment Period (AEP) between October 15 and December 7, your coverage will become effective January 1 of the following year.

Best Available Evidence (BAE)

Medicare Advantage Plans, like CarePlus, must accept certain forms of evidence of Extra Help or Low Income Subsidy (LIS) assistance when our system or the CMS system does not indicate that a beneficiary is eligible for this assistance.
