In 2018, CarePlus offers MAPD plans in many counties throughout Florida.

Please check the links provided below to review the Summary of Benefits and/or Evidence of Coverage (EOC) for each specific Medicare Advantage plan, which will provide you with the plan's description of benefits.

You must use plan providers, except for emergencies, out-of-area urgently needed care, or out-of-area dialysis services, and cases in which CarePlus authorizes use of out-of-network providers. For in-network urgent care needs, member should go to network providers. You must receive all routine care from plan providers. If you obtain routine care from out-of-network providers without prior authorization, neither Medicare nor CarePlus will pay for the services.

This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. Benefits, premiums and/or member cost-share may change on January 1 of each year.
