C-SNP stands for chronic condition special needs plan. You must have a qualifying chronic condition to enroll.
If you have a condition like diabetes, heart disease or asthma, a C-SNP may help cover the costs for specialist visits and medications you need for your condition. CarePlus Medicare Advantage C-SNPs also include extra benefits like coverage for acupuncture, routine dental care, eyewear, hearing aids and much more.
Several of our CarePlus C-SNPs for 2025 and 2026 include a Medicare Part B premium giveback benefit. This benefit covers all or part of your monthly Medicare Part B premium payment. That means more money in your Social Security check every month.

CarePlus offers four C-SNPs for 2026: CareComplete (HMO C-SNP), CareComplete Platinum (HMO C-SNP), CareBreeze (HMO C-SNP) and CareBreeze Platinum (HMO C-SNP). Each C-SNP offers slightly different benefits tailored to support people with specific chronic medical conditions.
CareComplete is a C-SNP for people with diabetes, heart disease or a qualifying cardiovascular condition. It offers special programs to help manage your chronic condition(s) and may reduce your costs for certain medications. This plan includes a spending card allowance benefit and is available in all CarePlus service areas.
CareComplete Platinum is a C-SNP for people with diabetes, heart disease or a qualifying cardiovascular condition. It offers special programs to help manage your chronic condition(s) and may reduce your costs for certain medications. This plan includes the Part B giveback benefit and is available in all CarePlus service areas.
CareBreeze is a C-SNP for people with a qualifying chronic lung disorder such as asthma, bronchitis or emphysema. It offers special programs to help manage your chronic condition(s) and may reduce your costs for certain medications. This plan includes a
CareBreeze Platinum is a C-SNP for people with a qualifying chronic lung disorder such as asthma, bronchitis or emphysema. It offers special programs to help manage your chronic condition(s) and may reduce your costs for certain medications. The Part B Premium Reduction Benefit pays part or all of your Part B premium, and the amount may change based on the amount you pay for Part B.
CareComplete (HMO C-SNP): This plan is available to anyone enrolled in Medicare with a diagnosis of diabetes, cardiovascular disorders or chronic heart failure.
CareComplete Platinum (HMO C-SNP) and (HMO-POS C-SNP): This plan is available to anyone enrolled in Medicare with a diagnosis of diabetes, cardiovascular disorders or chronic heart failure. In Broward and Palm Beach, CareComplete Platinum (HMO-POS C-SNP) covers certain services received from out-of-network providers located within the plan's service area. Except in emergency or urgent situations, non-contracted providers may deny care. You will pay a higher copay for services received by non-contracted providers.
CareBreeze (HMO C-SNP): This plan is available to anyone enrolled in Medicare with a diagnosis of chronic lung disorders.
