2018 Annual Notice of Changes and Evidence of Coverage

CarePlus members may refer to their Annual Notice of Changes (ANOC) and Evidence of Coverage (EOC) for plan information. The combined ANOC/EOC is sent to you every year in late September.

The ANOC tells you about any changes to your plan for the next plan year. In it you’ll find charts that show you how your premiums, deductibles, medical benefits and prescription drug benefits will change for the next year. It also tells you about other important changes to your plan. It’s important that you review this document thoroughly and soon after you receive it.

The EOC provides you with all the details of your plan for the new year beginning January 1st. It includes full, legal descriptions of your benefits and costs and explains how to access your benefits. You should also review this document thoroughly and keep it handy for reference throughout the plan year. If your plan is not listed, please call Member Services at 1-800-794-5907; from 8 a.m. to 8 p.m., 7 days a week. From February 15th to September 30th, we are open Monday-Friday from 8 a.m. to 8 p.m. TTY users should call 711..

Covered Durable Medical Equipment Items and Brands

Durable Medical Equipment (DME) is certain medical equipment ordered by your doctor for medical reasons. Generally, CarePlus covers any DME that is covered by Original Medicare if considered medically necessary. The list below tells you which brands and manufacturers of DME are covered under our benefit plans.
