CarePlus Healthy Foods Card program

This program is only available to Dual Eligible Beneficiaries that are enrolled in a D-SNP plan. What is a Dual eligible beneficiary? A person that qualifies for both Medicare and Medicaid benefits.

As part of your CareNeeds Plus (D-SNP) plan, you get a Healthy Foods Card with a monthly allowance to buy approved healthy foods and beverages. You can only use your card at participating retail stores or to buy a variety of pantry items online at Funds will be added to your card on the first of each month. Be sure to use your allowance each month. Unused funds do not carry over to the following month or plan year. This benefit ends on December 31 or upon disenrollment from the plan.

Purchase Items like:

  • Bakery - breads, tortillas, rolls, etc
  • Beverages - coffee, tea and juice
  • Dairy - milk, eggs, cheese, yogurt, etc
  • Frozen - produce, meals, vegetables, etc
  • Grab and go - fresh meal kits and prepared deli items
  • Meat and seafood
  • Pantry - baking goods, canned goods, nutritional shakes, pasta, snacks, etc
  • Produce - fresh fruits and vegetables, packaged salads, etc

Three ways to shop with your Healthy Foods Card:*

  • In-store – Shop with your Healthy Foods Card at participating stores near you. You can also use the store finder online: (link opens in new window).
  • Online - Order online at to have approved pantry items, like canned goods, rice and pasta delivered to your home. You now have the ability to order shelf-stable foods through Shelf-stable foods include:
    • Baking items
    • Beverages
    • Boxed meals
    • Cereal
    • Canned goods
    • Canned soup
    • Condiments, sauces, and spices
    • Coffee
    • Grains and pasta
    • Snacks, cookies, and chips

*Your benefit only covers approved items up to the amount on your card. You can use another form of payment to cover any remaining balance or items not approved.

How to get started

  1. Activate your card online or by phone. Call 1-833-832-7306 (TTY: 711). You’ll need to enter your card number, birthdate (MM/DD/YEAR), and zip code.   2. Shop in-store or online for approved foods and beverages. Use the online shopping guide included to help you shop for approved pantry items at and have them delivered.
  3. Pay at checkout, use your Healthy Foods Card FIRST to pay for all approved items. Have the cashier scan the barcode on the back of your card before any other form of payment.

Healthy Benefits Plus mobile app

To make in-store shopping easy and convenient, use the Healthy Benefits Plus mobile app to shop and pay for approved foods.

Steps for using the app

  • Download the Healthy Benefits Plus mobile app on your smartphone. The app is free in the App Store® or Google Play®.
  • Login with your username and password or register your card if you are a new user.
  • Tap ‘My Account’ to check your balance.
  • Tap ‘My Card’ to access the mobile barcode that can be scanned at the grocery store checkout.
  • To search for available retailers near you tap ‘Find Store.’ Start saving on healthy food!

  • Which plans include this benefit?
    All CareNeeds PLUS (D-SNP) plans include this benefit. The amounts vary by plan.
  • Where can I use their card?
    You can use your card in-store at Walmart, CVS (including Navarro Pharmacies), and Walgreens. You can also buy a variety of pantry items online at or ready-to-eat prepared meals from (link opens in new window).
  • What items are approved to buy with the Healthy Foods Card?
    Most non-alcoholic beverages and foods are approved (not including pet food).

    Here’s a list of the types of items that can be purchased by members who have this benefit as part of their plan:

    Perishable foods:
    • Dairy (milk, cheese, butter, sour cream, whipping cream)
    • Whole eggs and egg substitutes
    • Yogurt
    • Margarine
    • Soy and other nut milks
    • Lard
    • Ready-to-eat soups, meals, and sandwiches
    • Deli items—processed and fried
    • Produce (fruits and vegetables)

  • Are there items I cannot buy with my card?
    Yes, there are some non-food items you CANNOT buy with your card:
    • Alcohol
    • Fresh flowers or floral items
    • Household, cleaning, or personal hygiene products
    • Lottery or gaming tickets
    • Over-the-counter (OTC) items
    • Tobacco and vaping products
    • Pet food and pet supplies
    • Self-care items
    • Medicare prescription drugs and services
  • Can I use this card with other store discounts or food assistance?
    Yes, this card works with store discounts, coupons and other assistance you may have.
  • Do I pay for shipping for orders?
    Orders over $35 ship free. A $5.99 shipping fee applies to orders under $35. You cannot use your Healthy Foods Card to pay for shipping costs. There are no shipping charges on orders over $35.
  • How often can I shop?
    You can shop whenever you like and be sure to use the full amount on your card by the end of each month. Unused funds will not carry over to the next month.
  • What if my allowance isn’t enough to cover the items at check out?
    If you spend the full amount on your card and still have items to buy, you may use other forms of payment to complete the purchase.
  • Do I pay sales tax?
    Yes, you pay sales tax on these items just as you would normally.
  • Can I return purchased items?
    Purchased items are subject to the store’s return and exchange policies. All approved refunds will be credited to the balance of the card and must be used within 30 days of refund.
  • When should I expect to get my monthly allowance added to my card?
    Funds are added and available on the first day of each month. You must use your allowance by the last day of the month - unused funds will not carry over to the next month.
  • How can I check my card balance?
    In one of three ways:
  • What happens if my card is lost or stolen?
    If your card is lost, stolen or damaged, report it immediately. You may request a replacement card at (link opens in new window), by calling 1-855-391-0942 (TTY: 711), or on the mobile app if you have registered for an account.
  • How will I know if my benefits change from one year to the next?
    CarePlus will notify members of changes to the Healthy Foods Card as part of the Annual Notice of Changes (ANOC).
  • What if I have questions about my CarePlus Medicare Plan?
    Call CarePlus Member Services: 1-800-794-5907 (TTY: 711), 8 a.m. to 8 p.m. seven days a week from Oct. 1 – Mar. 31, and 8 a.m. to 8 p.m. Monday-Friday from Apr. 1 - Sept. 30. You may always leave a voicemail after hours, Saturdays, Sundays, and holidays and we will return your call within one business day.

No amounts on the Healthy Foods Card can be used to purchase Medicare-covered prescriptions or services, nor can it be converted to cash.
