Ways to order with CenterWell Pharmacy

Typically, you should expect to receive your OTC items within 10 to 14 business days from the time that CenterWell Pharmacy receives your order.

  • Mail

    Complete the OTC Mail Order Form and mail only the order form pages, using the postage-paid envelope provided, to:

    P.O Box 1197
    Cincinnati, OH 45201-1197

  • Fax

    Fill out the Over-the-Counter (OTC) Mail Order Form and fax only the order form pages to: 1-888-778-8384

  • Call

    Call CenterWell Pharmacy at 1-800-526-1490 (TTY: 711), Monday – Friday, from 11 p.m. or Saturday 8 a.m. to 6:30 p.m. to check the status of your order.
