Earn Rewards for Healthy Actions

Our CarePlus Rewards program is offered to all CarePlus members at no additional cost.

At CarePlus, you can earn a $50 gift card‡ just for taking care of your health! CarePlus is here to help you enjoy a rewarding journey. Every time you complete healthy actions, you will earn points. Get 50 points before the end of the year to earn your $50 gift card.

MyCarePlus lets you access your CarePlus Rewards program online. Track your activities and points right from your computer or smart phone. You will even get personal wellness tips along the way. When you earn enough points, you can order an e-gift card that is delivered instantly.

In MyCarePlus at CarePlusHealthPlans.com/logon, select CarePlus Rewards from the “My Resources” tab. Follow the prompts to register.

When you complete activities, points are automatically updated in our system. They will not show up right away, so please give it a little time. There is no need to call CarePlus, but we are here for you if you have questions.

If you do want to access your CarePlus Rewards program online, you can still participate. Once you have completed eligible activities, we will notify you by mail and provide instructions to redeem your gift card.

Complete at least one of these to earn 20 points. Each ☑ checkmark shows an activity that can be counted once per year toward your CarePlus Rewards.

  • “Welcome to Medicare” Visit (if you’re new to Medicare)

    This initial physical exam must take place within the first 12 months of getting Medicare Part B. This visit includes a review of your medical history and may include an electrocardiogram (EKG).

  • Annual Wellness Visit

    Your yearly physical exam after having Medicare Part B for 12 months

  • RealAge® Test

    Available with our CarePlus Rewards program online, or call Member Services to request it. This optional quiz calculates your body’s “health age.” and then helps you set goals to maintain or improve your health. Please note you are required to register for a digital tool called Sharecare either online or by phone, for this activity to count toward CarePlus Rewards.

Complete at least four of these activities to earn 30 points. Each ☑ checkmark shows an activity that can be counted once per year toward your CarePlus Rewards.

GENERAL HEALTH AND WELLNESS

  • Cardiovascular disease screenings

    Any one of the following counts for this screening: lipid panel, lipoprotein, cholesterol, or triglycerides.

  • Bone mass measurement

    Any one of the following counts for this screening: ultrasound bone mineral density measurement, dual-energy X-ray (DEXA), single-energy X-ray (SEXA), or radiographic absorptiometry

  • Depression screening

    Mental health is an important part of your well-being. Please talk to your doctor if you are feeling depressed or stressed out.

  • Medical nutrition therapy

    Nutrition counseling provided by a registered dietitian. Designed to help you select the right foods for your health.

VACCINES

  • Flu shot

I’m having trouble creating my digital rewards program account on Sharecare. How can I get help?

Our Member Services department can also help you. Please call Member Services at 1-800-794-5907 (TTY: 711). From October 1 - March 31, we are open 7 days a week, 8 a.m. to 8 p.m. From April 1 - September 30, we are open Monday - Friday, 8 a.m. to 8 p.m. You may always leave a voicemail after hours, Saturdays, Sundays, and holidays and we will return your call within one business day.

Where do I see my CarePlus Rewards in Sharecare?

Access our digital rewards program by going to Sharecare in MyCarePlus. In Sharecare, click the “Achieve” icon and then select “Rewards.”

Is there a cost to participate?

No. CarePlus Rewards is included as part of your CarePlus Medicare Advantage plan.

Do I have to participate in the CarePlus Rewards program?

No. You are not required to participate. We encourage you to participate, but it is your choice and completely optional.

How many rewards can I receive?

You can receive a total of one $50 gift card in a plan year. Earn 50 points by completing eligible activities to receive your $50 gift card.

Where can I use my $50 gift card?

We offer a variety of gift cards for different retailers and restaurants. When you order your gift card, you can choose one from our list of options.

How do I know which activities I can complete to earn rewards?

Our digital rewards program lets you can see which eligible activities you have completed and the amount of points you have earned. To create your digital rewards program, go to Sharecare in MyCarePlus. The first time you use our digital rewards program, Sharecare will prompt you to enter your Member ID number. If you need additional assistance is needed, please call Member Services at 1-800-794-5907 (TTY: 711).

How will I know if I earn a reward?

You can track your progress by using our digital rewards program. You need 50 points to earn a reward. After you complete your enough eligible healthcare activities, we'll also mail you a notification that includes ordering instructions for your gift card. If you’re not sure which activities you completed, call Member Services.

Do I have to tell CarePlus when I complete healthcare activities that qualify for rewards?

No. You do not need to tell us you completed a healthcare activity. Your doctor will submit that information to us after your visit. Please allow some time for it to be updated in our system.

How do I order my reward?

You can order your gift card by mail, fax, phone, or online through our digital rewards program. Once you’ve earned 50 points by completing eligible healthcare activities, we'll mail you a notification that includes ordering instructions for your gift card.

Do I pay anything when I order my reward?

No. All rewards are shipped or emailed directly to you free of charge.

Do I have to order my gift card immediately?

No. However, we must receive your gift card order by December 31st of the plan year. You must redeem your reward by this date or you will lose it.

How long will it take to receive my reward?

To receive your gift card by mail, please allow 10 to 14 days for delivery of your reward from the date it is received and processed. It will be shipped by the US Postal Service. Want your gift card faster? When you use our digital rewards program, you may choose electronic delivery to receive an e-gift card right away through email.

Can I cancel my reward order?

Once you order your reward, it is final and cannot be cancelled.

Can I change my order after it has been placed?

No, all orders are final and cannot be changed.

Who can I call if I have questions about my reward?

For questions about your reward order or the CarePlus Rewards program, please call Member Services at 1-800-794-5907 (TTY: 711).

In accordance with the federal requirements of the Centers for Medicare & Medicaid Services (CMS), no amounts on the gift cards shall be redeemable for cash and no amount may be applied toward the purchase of any prescription drug under your plan. Rewards (gift cards) must be earned and redeemed in the same plan year. Rewards not redeemed by 12/31 will be forfeited.
