Because CarePlus Medicare Advantage benefit plans are designed to help you stay healthy, we've developed programs and services that can help you focus on wellness and prevention, in order to promote healthy aging and help keep you active.

Disease Management Programs

CarePlus' disease management programs can help you manage certain chronic medical conditions. We keep track of the latest research on treatments and make sure you're informed. Using medical science and statistics together, we strive to identify members who are at risk for contracting conditions like diabetes, even before the symptoms surface.

Where Good Health Begins

Most beneficiaries and healthcare professionals would agree that good health begins with a proactive lifestyle; eating nutritious foods, not smoking, exercising regularly, good personal hygiene habits such as hand-washing, reducing stress and obtaining restful sleep. But here's another proactive measure to consider: preventive health screenings and vaccines.

Which Screenings Do You Need?

Screenings can help uncover diseases early when they are easier to treat. Below is a list of recommended screenings based on the United States Preventive Service Task Force (USPSTF) guidelines for most adults.

An earlier start to, or additional screenings may be recommended by your physician depending on your health, age, race, gender, family history, and lifestyle. Why not schedule an appointment with your doctor to discuss preventive screenings?

Social Services Department

The CarePlus Social Services department helps members apply for public assistance. They work with programs like the Medicare Savings Program (coordinated with the state Medicaid program) and the Extra Help prescription drug assistance program among others. CarePlus has been helping members with their Medicare/Medicaid dual eligibility status since 2002. Using this service is completely voluntary. If you choose not to use this service, your enrollment into a CarePlus plan will not be affected.

The specific agency that offers the options for assistance will be the one responsible to determine your eligibility. You are also free to apply for public assistance directly with the sponsoring agency.
