Physicians and other healthcare providers can request to join the CarePlus network of providers by completing and faxing the
CarePlus contracts with Carelon Behavioral Health to manage its network of providers to render substance abuse and mental health counseling and treatment to our members. We encourage coordination of care, communication, and information exchanges between behavioral health and medical practitioners.
For convenient phone or video chat triage, diagnosis, treatment, and prescriptions, you can refer your CarePlus-covered patients to CareNow® for all covered services except behavioral health.* To access CareNow® , CarePlus members can call My Home Doctor at

CarePlus members use our provider directories to find and contact participating physicians and other healthcare providers. Stay connected by updating your listing if there are changes, such as a new address, phone or fax number, website, or if you start or stop accepting new patients. To update your information, contact your provider services executive by fax at 866-449-5668 or call the CarePlus Provider Operations inquiry line at 866-220-5448, Monday to Friday, 8 a.m. to 5 p.m., Eastern time. If prompted to leave a message, please include your contact information so we can easily reach you.

CarePlus is an HMO plan with a Medicare contract. Enrollment in CarePlus depends on contract renewal.

Hours of operation:
From October 1 - March 31, we are open 7 days a week, 8 a.m. to 8 p.m. From April 1 - September 30, we are open Monday - Friday, 8 a.m. to 8 p.m.
You may always leave a voicemail after hours, Saturdays, Sundays, and holidays and we will return your call within one business day.
