CarePlus Medicare Advantage Dual Eligible Special Needs Plans (D-SNPs) include extra benefits like free transportation to medical appointments and an allowance on a spending account card. You must have Medicare and Medicaid to enroll in a D-SNP.

What’s a D-SNP?

D-SNP stands for Dual Eligible Special Needs Plan. It is a special type of plan that coordinates your Medicare and Medicaid benefits in one easy-to-use plan. It includes coverage for doctor visits, medication, dental, vision and much more.

Medicare Advantage D-SNPs usually include extra benefits that you wouldn’t get from Original Medicare or Medicaid alone.

Who’s a good fit for this plan?

To enroll in a D-SNP, you must have both Medicare and Medicaid.

Qualifying conditions include diabetes mellitus, cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, or chronic heart failure, among others. Some plans require at least two conditions and other requirements apply. See the plan's Evidence of Coverage for details. If you use this program for rent or utilities, Housing and Urban Development (HUD) requires it to be reported as income if you seek assistance. Contact your local HUD office if you have questions.  

CarePlus is a Dual Eligible Special Needs HMO SNP plan with a Medicare contract and a contract with the Florida Medicaid Program. Enrollment in this CarePlus plan depends on contract renewal.

CareNeeds Plus (HMO D-SNP) and CareNeeds Platinum (HMO D-SNP): Depending on where you live, these plans may be available to anyone receiving both Medicare and Medicaid: Qualified Medicare Beneficiaries (QMB/QMB+), Specified Low-Income Medicare Beneficiaries (SLMB/SLMB+), Qualifying Individuals (QI), Qualified Disabled and Working Individuals (QDWI) and other Full Benefit Dual Eligibles (FBDE).

CareNeeds Extra (HMO D-SNP): This plan is available to anyone receiving both Medicare and Medicaid covered services: Qualified Medicare Beneficiaries with Medicaid (QMB+), Specified Low-Income Medicare Beneficiaries with Medicaid (SLMB+), and other Full Benefit Dual Eligibles (FBDE). 

 No amounts on the CarePlus Spending Account card can be combined or converted to cash.
