What’s a D-SNP?

D-SNP stands for dual eligible special needs plan. It is a special type of plan that coordinates your Medicare and Medicaid benefits in one easy-to-use plan. It includes coverage for doctor visits, medication, dental, vision and much more.

Medicare Advantage D-SNPs usually include extra benefits that you wouldn’t get from Original Medicare or Medicaid alone.

Who’s a good fit  for this plan?

To enroll in a D-SNP, you must have both Medicare and Medicaid.

Medicaid is generally for people with limited income and assets. To see if you are eligible for Medicaid, you are welcome to call CarePlus Social Services.

You have options.

For 2025, CarePlus offers two D-SNPs: CareNeeds Plus (HMO D-SNP) and CareNeeds Platinum (HMO D-SNP). For 2026, CarePlus offers three D-SNPs:  CareNeeds Plus (HMO D-SNP), CareNeeds Platinum (HMO D-SNP), and CareNeeds Extra (HMO D-SNP). They offer many of the same benefits but with a couple of key differences. Keep in mind that if you receive Extra Help, you’ll pay $0 for all covered prescription drugs on all three plans.

  • CareNeeds Plus

    CareNeeds Plus has more dental coverage than CareNeeds Platinum. It is available for people with any level of Medicaid, but it is not available in Miami-Dade, Broward or Palm Beach.

     

  • CareNeeds Platinum

    This plan has a higher spending card allowance than CareNeeds Plus. It is available for people with any level of Medicaid and is offered in all CarePlus service areas.

     

  • CareNeeds Extra

    This plan is only available for people who have full Medicaid. If you qualify for this plan, it offers an even higher spending card allowance than CareNeeds Platinum. CareNeeds Extra is offered in all CarePlus service areas.
