Have Medicare and Medicaid?
CarePlus Medicare Advantage Dual Eligible Special Needs Plans (D-SNPs) include extra benefits like free transportation to medical appointments and an allowance to spend on healthy food, utility bills and more. You must have Medicare and Medicaid to enroll in a D-SNP.

What’s a D-SNP?

D-SNP stands for dual eligible special needs plan. It is a special type of plan that coordinates your Medicare and Medicaid benefits in one easy-to-use plan. It includes coverage for doctor visits, medication, dental, vision and much more.

Medicare Advantage D-SNPs usually include extra benefits that you wouldn’t get from Original Medicare or Medicaid alone.

Who’s a good fit  for this plan?

To enroll in a D-SNP, you must have both Medicare and Medicaid.

Medicaid is generally for people with limited income and assets. To see if you are eligible for Medicaid, you are welcome to call CarePlus Social Services.
